The Waianae Coast Comprehensive Health Center is honored to announce it has received Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) for its Main Center, James and Abigail Campbell Nanakuli Clinic, Waipahu Family Health Center, and West O'ahu Community Health - Kapolei.

NCQA accredits and certifies a wide range of health care organizations and manages the evolution of HEDIS®, the performance measurement tool used by more than 90 percent of the nation's health plans.

NCQA's recognition programs are built on evidence-based, nationally recognized clinical standards of care. The PCMH standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement.

Through implementing medical home functions, practices seek to improve the quality, effectiveness, and efficiency of the care they deliver while responding to each patient’s unique needs and preferences.

NCQA's PCMH Recognition Program is the most widely adopted evaluation program in the country. Approximately 13,000 practices (with more than 60,000 clinicians) are recognized by NCQA. In Hawaii, 46 primary care practices are recognized under NCQA's prestigious PCMH program.

The recognition would not be possible without the clinical, care coordination, referral and medical receptionist teams working together to provide comprehensive and quality care to our patients.
